J-1: Change of Program, Degree Level, or Funding
Your Form DS 2019 indicates:
- degree program (major)
- degree level (bachelor's, master's, PhD)
- funding source
If at any time there is a change in these areas, a new Form DS 2019 indicating the change must be prepared by the ISS office. When requesting a change, submit a Student Request Form and submit with it the following:
- Form DS 2019
- proof of the change (a letter from your department or funding source is generally sufficient)
- passport
- Form I-94 card
